For the MD-PRO 64M on the old website the description says the USB and Parallel cable are optional, how do I order these and how much will they cost.

I had the cables, but bought ones from TotoTek anyway just so that I was using new cables and still have my old ones for other things. You can find both the parallel and usb cables on the old TotoTek site, same as the MD-Pro 64 (which is what I bought). If I remember correctly, they were like $5 and $3 or something close. Nothing outrageous. Just look for cables on the old site and you shouldn't have too much trouble finding them.
