Original Game Doctor III come with clone NTSC cic chip. So, your Super Famicom must be NTSC version.
And yes, you can use it as a adapter to play Pal game through it.
If your Game Doctor III buy from ToToTEK, then clone NTSC cic will change to universal cic and bios will upgrade to v3.3C. So, it can run on both PAL/NTSC SFC/SNES. |

No, you need to change ntsc cic to pal cic or universal cic. Then it can work on Pal SNES. |
